How can I schedule an appointment?

Appointments can be scheduled by calling our office. If you reach our voicemail, please leave a message and we will call you back as soon as we can. You can also fill out the form on our contact page to request an appointment. If you are a new patient, you can schedule a Free 20 Minute Consultation here.

Do you offer virtual appointments?

Yes! It is possible to schedule a virtual appointment with Dr. Berkeley. For established patients, you must be located within the District of Columbia or Maryland at the time of your virtual appointment.

I'm a new patient. How do I start working with you?

New Patients: A Free Virtual 20 Minute Consultation for New Patients is REQUIRED before booking a New Patient Visit. During this time, you may ask any questions you have regarding care and get to know Dr. Berkeley. This consultation is for informational purposes only and does not provide medical advice.  Schedule here.

Online Intake Form: You will receive a link to fill out an online intake form once your New Patient Visit is scheduled. The link for this form will be sent to you along with your appointment confirmation email. This form must be completed and signed at least 1 week before your appointment. Important: please sign the form by using your computer's mouse or your finger (if you have a touchscreen computer). You will also be given consent forms to sign electronically and these also must be completed and signed at least 1 week before your appointment.

New Patient Visit: During this appointment, we will spend time talking about your health history and factors that may be affecting your health. Our goal is to find the underlying conditions that may be preventing you from achieving your health goals. An exam and lab work will give us more information. A Review of Findings appointment comes after your New Patient Visit and we will discuss your individualized plan at that time (after lab work results have returned).

Review of Findings Appointment: During this appointment, we will review lab work results and you will receive the first steps towards your individualized plan.

Follow-up Appointments: We will discuss your schedule for returning for follow-up appointments at your New Patient Visit or Review of Findings Appointment.

What should I bring to my New Patient Visit?

(1) Please bring all current medications and supplements. (2) Please also bring a photo ID with you to your appointment. (3) Please complete an online intake form and consent forms. The online intake form and consent forms must be completed at least 1 week BEFORE your appointment.

What is your cancellation policy?

Please note that 2 business days notice is required for any changes to appointment times or to cancel an appointment. (For example, to reschedule or cancel an appointment scheduled for Tuesday at 10am, you should contact our office before 10am on the previous Friday).

Any rescheduling or cancellation requests with less than 2 business days of notice will result in a full charge for the originally scheduled appointment time. If you provide 2 or more business days of advanced notice, we will make every effort to reschedule your appointment as soon as possible.
